St. Thomas Facility Use

This form must be used to obtain permission to use the space in any of the parishes. Multiple use of a specific area may be requested at one time. Application for use request must be submitted at least 10 days prior to date and two weeks prior if custodial staff needs are requested. Submission of application does not guarantee use of space. You will be notified when approval is given.

There is a charge for non-church/outside groups to use our facilities. Please submit this form and someone from the Parish Office will be in contact with you. Submitting this form does not commit you to using our space.

A church key must be signed out at the parish office during business hours. If the key is lost, cost of lock replacement for Church is required.

All furniture and equipment must be returned to its designated place. Space must be cleaned and tidy prior to departure.
