Facility Use Form
St. Andrew’s & St. Mathias Facility Use
This form must be used by all Church staff and all volunteers to obtain permission to use space in any of the parish properties. Space requests MUST BE SUBMITTED at least 10 business days before the date of use. Submission of an application does NOT guarantee the use of space. You will be notified when approval is given. (If 10 business days' notice is not received, you will be responsible for the setup, take down, and cleaning of the space.)
Please note that ‘FOR PROFIT’ meetings/gatherings are not allowed at Church facilities.
If this is not a "church-related" event, please review the FACILITY RENTAL FEES section (below). All rental fees are due before the use of the facility. This section also lists the maximum occupancy numbers for each space.
If you are unable to meet on the requested date(s) please notify the parish office ahead of time to free the meeting space for others.
The use of facilities for Church purposes will have precedence over all other requests/use. • All scheduled activities must have adult supervision approved by the Pastor, Business Manager, or designee. The group using the facilities will be responsible for damage to the building, grounds, or equipment and any custodial fees incurred if required. • Groups receiving permission are restricted to the dates and hours approved and to the building area and facilities specified. • The use of alcoholic beverages is permitted with a City Alcohol Permit, licensed vendor, and pre-approval from Catholic Mutual (parish insurance carrier). Smoking is NOT permitted in buildings or on property. • Proper liability insurance will be required for all individual, private, or commercial purposes. The liability insurance shall be, as a minimum, $1,000,000 liability, the same as that amount carried by the Church facilities. The Parish and the Diocese of Duluth shall be named as the additional insured party. • No group, organization, or individual may sublet use of the building/facility to any other group. • Keys distributed must be returned immediately after the activity. Copies of keys are not to be made under any circumstances. • The Pastor or Business Manager reserves the right to cancel any permission granted. • Fees must be paid before use. • CATERERS (Church): Must bring only prepared food & their equipment. • Children must be supervised at all times by an adult.
