Request For Sacramental records
If you are in need of a copy of your sacramental records please complete the following form.
We are only able to locate and create certificates for sacraments that have happened at St. Francis, All Saints or St. Thomas of The Pines.
I was Baptized there, but moved away!
If you were Baptized at one of these churches and then moved away, it is likely we will have a complete list of your sacraments; However, in the case that some sacraments are missing you will need to contact the Church of which you received that Sacrament and request they notify us. We can only provide you with what we have recorded. After receiving notification from the other church(s) we will be able to record your sacrament and provide a complete certificate to you.
How long will it take?
Please note it may take up to two weeks to verify your information.
How can I get this certificate?
Once all information has been verified we will mail the the certificate to the address you provided. You may also pick up in the Parish Office.
